abdominal hysterectomy cpt codes/ICD-9/modifiers???

Surgeon performed diagnostic laparascopy revealing lrage abdiminal mass, biopsied and confirmed as malignant. Total hysterectomy was done w/ total pelvic lymphadenectomy.  Lymph mode samplings were done and a BSO. 

Any help would be GREATLY appreciated,
