Betty, I feel it would be 27134 or 27137 or 27138 based upon the scenario with dx as 996.66.
I would appreciate if Barb could give her comments on the codes which I've suggested.

i forgot to tell about the modifier. since this is the second stage of the procedure i would suggest modifier 58.
