New Patient vs Established Patient

We are a stand alone urgent care facility.
If a patient is seen by an internist in the last three years but returns and is then seen by a family practitioner should that patient be considered a "new patient"
or is it considered type B provider-based emergency care and therefore no distinction should be made between new and established?
