I am taking (and I think acing!) a coding test for an ER coding job. They have hit me with one stumper--I think just because I don't know what they mean! They deal with the 1995 guidelines (yuck) and refer to an "acuity caveat rule which I've never even heard of! Can someone tell me what they mean? Also--the question is whether it may apply to the history, the exam, or both
