Proper modifier useage with multiple eyelid excision 67840 ( with 67904
When coding 67904,

with 4 eyelid excisions,
1 upper lid, 3 lower lid lesions
wouild it be shown as 67904,
67840-E1 1 upper lid
67840,59,E2 lower lid
67840,59,E2 lower lid
67840,59,E2 lower lid
Is this correct wayh to show modifiers?

2. When would one use 67840 vs begnign eyelid excision codes? 11440-11446 ?

Per the guidelines in CPT: 67840 is used when the removal of the lesion involves more than skin (i.e. involving lid margin, tarsus, and/or palpebral conjunctiva).

If the removal of the lesion involves mainly skin of the eyelid, you'd use the appropriate 11310-11313, 11440-11446, 11640-11646, 17000-17004 codes.

Remember to always pay attention to the guidelines in the same area as the codes your looking at, not just at the front of the section.

Your choice of codes is going to depend on what the provider indicated he/she performed...was it mainly skin or a more involved procedure. You  then code accordingly.
