Looking for a ICD-9 code for a "past spinal injury". Any suggestions would be greatly appreciated.

You might try V15.59

Or you could choose a Late Effect code if the patient is experiencing problems due to that previous injury. e.g back pain  (the back pain as a late effect of the spinal injury).
