I have billed a 99213 with a modifier 25 the diagnosis was 386.12  then I billed 69210 removal of impacted cerumen with dx for impacted cerumen and then an EKG 93000 for high blood pressure. Apparently the EKG is bundling with the office visit and I was told I need a modifier for the EKG but I have no idea what. Any help would be appreciated

You can try billing this way Ann.

99213-25  -  386.12

69210  -  380.4

93000-59  -  401.9
